Our mission is to help and enable people to manage their Diabetes and Hypertension in all aspects of their daily life. We utilize hardware and software alongside a readily available care team to provide a well-rounded solution to help patients outside of their Doctors appointments.

Solutions

iHealth Unified Care - Chronic Care Management Platform

Our care teams are comprised of Certified Diabetes Educators, Registered Dietitians, and Medical Assistants that work side by side with Primary Care Physicians (PCP) to create a new medical model for the treatment, management and adherence of Diabetes and Hypertension patients.

Unified Care focuses on the wholistic care of patients inside and outside of the Doctor’s clinic. We connect with patients in the Doctors clinic, helping them get familiar with the program and vitals measurement devices and continue that relationship beyond the appointment. We provide a connection with our care team to provide timely health related answers, as well as, food and vitals measurement reviews to help our patients make lifestyle adjustments that are specific to them.

As we continue to empower and support our patients, they have shown great strides in improving their overall health!

We had a great show this year at CES, as we do every year. We brought with us a line of our mobile health products that attendants are always excited to get hands-on time with. Our new touch-free thermometer was especially a hit at this year’s show.
